Kids Vacation Itinerary

If you and your husband are planning a private vacation, consider making your kids their own itinerary while you are gone.

Be sure to include:

1. Emergency contact numbers
2. A copy of your insurance cards
3. Signed copy of the authorization for consent to medical treatment for minors form.
4. Be sure to write on there where will documents (and other legal forms) could be found in case of an emergency.
